Lost or Damaged Plates If your license plates are lost, stolen or damaged, this can be resolved in one of two ways: 1. Plate Change You will be issued a new plate number from our stock plates. Click here for information on Plate Change. 2. Plate Remake You will retain the same plate number. This transaction can be performed online or by mail. Perform this transaction online here: Plate Remake / Plate Design Change Order a remake of your existing plate or order your plate in a different design. To perform this transaction by mail: Please fill out the application for plate remake. Mail the Application for Remake of License Plates form with the appropriate payment to the Division of Motor Vehicles Attn: Plates, 600 New London Avenue, Cranston RI 02920. Vehicles issued two plates: If both plates are lost/stolen, you need to provide a police report or complete the Affidavit for Cancellation of Registration for Lost Plates. You can file for the report at your local law enforcement agency. Vehicles issued one plate: You must provide a police report for the lost/stolen plate or complete the Affidavit for Cancellation of Registration for Lost Plates. There is no charge for replacement stickers.
