GU-1338 Rhode Island Special Financial Responsibility Insurance Certificate

A GU-1338 Insurance Certificate is required for certain businesses, plate types and anyone under the age of 18 registering a vehicle in their name. Below is a list of the required types:

Auto Body, Bailee Plates, Dealer Plates, Driving Schools, Fuel Delivery vehicles, In-transit Plates, Leasing Companies, Minors, Motor Vehicles for hire, School Buses, Public Service vehicles (including taxis), and Transporter Plates.

GU-1338s are obtained by contacting an insurance company that is authorized to do business in the State of Rhode Island.

All GU-1338's must be on record with the Cranston DMV’s Financial Responsibility Office. They can be emailed to DMV.FinancialResponsibility@dmv.ri.gov, faxed to 401-462-5805, or mailed to: DMV - Financial Responsibility, 600 New London Avenue, Cranston, RI 02920.

Minors (anyone under the age of 18) are required to file a GU-1338 to register a vehicle in their name.

The filing must include their full name, date of birth and the year, make, model and the entire identification number of the vehicle.

Canceling a GU-1338 requires an SR-26 cancelation form issued by the insurance company with the date of cancelation provided.  If only removing one vehicle from the policy, the year, make, model and the entire identification number of the vehicle must be listed on the form.

Once an SR-26 has been processed all vehicles and plate types will be canceled effective on the date of the cancelation.  If a new policy is not received prior to the date of cancelation a reinstatement fee will be required.

Individuals who are no longer minors may cancel their GU-1338 requirement by having their insurance company submit an SR-26, stating they are no longer a minor, or by notifying the Financial Responsibility Office by email at DMV.FinancialResponsiblity@dmv.ri.gov, or by calling the office at 401-462-9246.

Please be advised that Financial Responsibility (SR-22) is no longer required by the State of Rhode Island.
